How do I get a USCAP username and password

1. At the far right on the top (orange) toolbar on the USCAP website, click the blue “MyUSCAP” button.

2. Here you may apply for USCAP membership to get a member’s login (first blue link below the Submit button), or if you simply want to request a USCAP userid, click the second link below the Submit button.

How do I reset my password?

1. At the far right on the top (orange) toolbar on the USCAP website, click the blue “MyUSCAP” button.

2. Here you will be prompted to Login with your USCAP username and password.  

3. Once you login, look in the lower right portion of the screen which gives you links into the Membership System. 

4. There you can click “Change My Username or Password”.

I forgot my username and password how can I get this information?

1. At the far right on the top (orange) toolbar on the USCAP website, click the blue “MyUSCAP” button.

2. Here you will be prompted to Login with your USCAP username and password.  

3. If you have forgotten your username and/or password, click the third blue link below the Submit button to have it sent to you.

How do I change my address? Email? Phone #?

1. At the far right on the top (orange) toolbar on the USCAP website, click the blue “MyUSCAP” button.

2. Here you will be prompted to Login with your USCAP username and password.  

3. Once you login, look in the lower right portion of the screen which gives you links into the Membership System. 

4. There you can click “Change My Address/Personal Information”.

Where do I find other members' addresses?

1. At the far right on the top (orange) toolbar on the USCAP website, click the blue “MyUSCAP” button.

2. Here you will be prompted to Login with your USCAP username and password.  

3. Once you login, look in the lower right portion of the screen which gives you links into the Membership System. 

4. There you can click “USCAP Membership Directory”.

How do I access the journal online?

1. On the top (orange) toolbar on the USCAP website, click “Membership”. 

2. Click “Journals” to access the journal(s) to which you are subscribed.

How can I access the online courses?

1. On the top (orange) toolbar on the USCAP website, click “eLearning”

2. You will be presented with the latest list of Online Courses available for purchase. 

3. Select a course to purchase.

4. Click the “Purchase” icon in the rightmost column next to the desired course.  There you will be able to purchase the course.

5. You will then be returned to “My Online Courses” where you will be able to view the course, then evaluate, take the SAM test and claim credit.

How long is my online course available?

1. On the top (orange) toolbar on the USCAP website, click “eLearning”.

2. You will be presented with the latest list of Online Courses available for purchase. 

3. Select the link near the top to the left titled “My Online Courses”.

4. You can review the column titled “CME Expiration Date” to determine how long your online courses are available.

How do I apply for membership?

1. On the top (orange) toolbar on the USCAP website, click “Membership”.

2. Then click the “Apply for Membership” button.

3. You can select the “Online Membership Application” button and submit

OR

    You can click “Membership Application” button to print out the form to be mailed or FAXed to USCAP.

Where do I renew my membership?

1. At the far right on the top (orange) toolbar on the USCAP website, click the blue “MyUSCAP” button.

2. Here you will be prompted to Login with your USCAP username and password.  

3. Once you login, look in the lower right portion of the screen which gives you links into the Membership System. 

4. There you can click “Pay USCAP Membership Dues”.

How much are membership dues?

1. On the top (orange) toolbar on the USCAP website, click “Membership” and scroll down to see the current rates.

Where do I find my expiration date for my membership?

1. At the far right on the top (orange) toolbar on the USCAP website, click the blue “MyUSCAP” button.

2. Here you will be prompted to Login with your USCAP username and password.  

3. Once you login, look in the lower right portion of the screen which gives you links into the Membership System. 

4. There you can click “Maintain My Member Profile”.   

5. Member expiration date is listed near the bottom of the list. 

How do I print a past receipt for membership? A meeting?

1. At the far right on the top (orange) toolbar on the USCAP website, click the blue “MyUSCAP” button.

2. Here you will be prompted to Login with your USCAP username and password.  

3. Once you login, look in the lower right portion of the screen which gives you links into the Membership System. 

4. There you can click “View Recently Placed Orders”. 

5. Select the orders placed by accessing the Go Button dropdown that allows you to select a timeframe.  

6. See all orders placed in that timeframe.  

7. Receipts can be printed from here, but note:  the only bona fide receipt you receive was presented when you placed the order.

When and where is the next annual meeting?

1. At the far right on the top (orange) toolbar on the USCAP website, click the blue “Meetings” button.

2. Scroll to review the highlighted meetings and the next Annual Meeting information.

How do I add courses to my registration?

You must call USCAP support at 706.733.7550 to accomplish this.

How do I complete my evaluations online? 

1. The most direct route to deal with all of your requirements around any meeting is to click the blue “MyUSCAP” button at the right side of the orange toolbar. 

2. Next you will be prompted to Login with your USCAP username and password. (If you do not have one, or have forgotten, See #1 or #3 above). 

3. After logging into MyUSCAP, on the left side of the screen, you will see all the active USCAP meetings –
 Current (if there are any in session right now), Upcoming meetings and Past Meetings. 

4. Scroll down to the meeting you want to evaluate and click “Evaluations and CME/SAM Claims”. Note: if you do not see this prompt under the meeting you want to evaluate, the meeting either has not taken place yet – or evaluations are closed. Typically, you must evaluate a meeting within 6 months of the event. 

5. After the Evaluation Menu appears, you must complete up to 4 steps to claim credit. 

  •     First, you must evaluate each session you attended by clicking Step #1. 
  •     Next you must give us an overall evaluation of the entire meeting. 
  •     Optionally, if there are SAM tests for this meeting, you may take the SAM Test(s) in Step #3.
  •     Finally, in Step #4 (these last two steps only appear after you complete Steps #1 and #2) you review your CME/SAM Claim form, make any corrections.  

6. Then click the Transmit button to have the credits applied to your portfolio.

How can I claim my CME/SAMs credits?

1. First you must follow all the steps in #18 to get to the Evaluation Menu for the meeting in question. 

2. We encourage you to click the “Instructions” button at the top of the Evaluation Menu and read details of how to enter evaluations, take SAM tests, and claim your credit.

How do I print my CME/SAMs portfolio online?

1. At the far right on the top (orange) toolbar on the USCAP website, click “MyUSCAP”.

2. You will be prompted to Login with your USCAP username and password.  (If you do not have one, or have forgotten, See #1 or #3 above).

3. After logging in to MyUSCAP, on the lower right hand side of the screen you will see links to the Membership System.

4.  Click the link: CME/SAM Portfolio and there click the “Print Official Transcript” button. 

Where do I find handouts from past meetings? E.g. meetings from 2012, etc?

1. At the far right on the top (orange) toolbar on the USCAP website, click the blue “MyUSCAP” button.

2. You will be prompted to Login with your USCAP username and password.  (If you do not have one, or have forgotten, See #1 or #3 above).

3. After logging into MyUSCAP, on the left side of the screen, you will see all the active USCAP meetings – Current (if there are any in session right now), Upcoming meetings and Past Meetings.

4. Scroll down to the meeting you want and look for the link: “Post-meeting Session Materials”.  

5. Click to display a list of all materials you are authorized to download from that meeting based on your registration and any ticketed sessions that you may have purchased, ex: Short Courses. 

What is your mailing address?

1. On the top (orange) toolbar on the USCAP website, click “About Us”.

2. Scroll to the bottom of this page where you will find the address of the East and West Coast Offices of USCAP. For your convenience we list them here as well:
