All fees are shown on the form.

You are encouraged to register early - the fee for advance general registration is less than it will be
for on site registration. All fees are shown on the form. The General Registration Fee is required for
admittance to any function of the Academy. This fee entitles you to attend the entire program on Monday
and Tuesday as well as the poster session on Wednesday and all the evening Specialty Conferences.
Additional fees are required for the Long Course, Special Courses and Short Courses. A single
registration fee of $80 is required for admittance to any one or all of the Companion Meetings which are
held on Saturday evening and Sunday, except the Society for Pediatric Pathology which will have its own
separate registration. Special reduced registration fees are shown on the registration form which apply
to USCAP Junior Members only. Short Course glass slides will not be mailed to those using the reduced
fee. Pre-Registration forms will be accepted until February 18th. Do not
submit forms to the USCAP office after that date - please utilize On-Site Registration at the meeting.
Please note that the On Site general registration fee will be higher than the Advance general
registration fee. Do not use the enclosed form for On Site Registration.
Refund Policy
Cancellations received prior to January 31, 2005 are subject to a $25 administrative charge and a $25
handling fee will be assessed for each short course cancelled if study materials were mailed. No refunds
will be made until short course study materials have been returned. Cancellations received after January
31, 2005 will be assessed an administrative charge of 25% of the total registration fee. No refunds will
be made on cancellations received after the meeting has started.
Continuing Medical Education Credits
The United States and Canadian Academy of Pathology is accredited by the Accreditation Council for
Continuing Medical Education to provide continuing medical education for physicians. The Academy takes
responsibility for the content, quality, and scientific integrity of this CME activity. The Academy
designates that a maximum of 55 hours in category 1 credit towards the AMA Physician's Recognition Award
is available during the annual meeting. Each physician should claim only those hours of credit that
he/she actually spent in the educational activities.

The Academy also jointly sponsors the programs of the following Companion Societies whose programs
have been planned and implemented in accordance with the Essential Areas and Policies of the
Accreditation Council for Continuing Medical Education through the joint sponsorship of the USCAP and
these societies.

Hospitality Program
A hospitality room will be open during the meeting and a variety of tours will be offered. The final
hospitality program will be mailed to all pre-registrants for the meeting with their receipt, and will be
posted on the USCAP website.
