This course is intended for general and surgical pathologists as well as for pathologists-in-training. It focuses on the identification and differential diagnosis of germ cell tumors and sex cord-stromal tumors of the testis, utilizing gross examination and light microscopy as well as genetic and immunohistochemical studies. There have been many new developments in this field in recent years. Cisplatin-based chemotherapy has greatly reduced the mortality from these neoplasms, but it has also changed the types and clinical presentations of the tumors seen. New patterns of tumors have become evident. The prognosis of individual tumors has changed and secondary neoplasms may complicate the clinical course. An awareness of the types of tumors seen in the era of modern chemotherapy as well as factors which may significantly alter the clinical course of disease will lead to more accurate diagnosis and optimal patient care.

A case presentation approach will be followed in the discussion of the following topics: Classification and pathogenesis of testicular germ-cell neoplasms (includes pediatric tumors), germ cell tumors of the adult testis, treatment associated lesions in men with testicular germ cell tumors and sex-cord-stromal tumors of the testis.

Upon completion of this course, participants will have an understanding of: 1) the different pathways of development of testicular tumors, 2) precursors to invasive tumors, 3) diagnosing and distinguishing different types of germ cell and sex cord-stromal neoplasms of the testis, with an emphasis on recently developed techniques, 4) changes in morphology and clinical presentation of these tumors secondary to cisplatin-based chemotherapy, 5) late recurrences of germ cell tumors, 6) nongerm cell neoplasms complicating the course of testicular neoplasms, and 7) criteria and techniques to diagnose different sex cord-stromal neoplasms. Differential diagnosis and the prognostic significance of different types of germ cell and sex cord-stromal tumors will be emphasized.
