This course is focused on how errors occur in diagnostic surgical pathology. The occurrence of errors in the different parts of the test cycle (pre-analytic, analytic and post-analytic) will be discussed. This includes specimen identification and handling, diagnostic errors due to lack of clinical correlation, misuse of ancillary studies, non-standard terminology, report construction and distribution. Examples of errors will be presented with discussion of how errors occur and how they may be mitigated.
